Two ways to pay.
Either way, in most cases you do not need a physician referral.
Paying yourself
- Private pay, HSA and FSA cards accepted
- A Super Bill for plans we don’t file with
- Call us for rates
Using insurance
- In network: Anthem Blue Cross Blue Shield, Tricare Select, Workers’ Comp
- Traditional Medicare, with a physician referral
- Most other plans filed out of network
